Skin graft after compartment syndrome in the forearm: case report

Background: Compartment syndrome is an emergency clinical condition characterized by increased pressure in a muscle compartment, which compromises blood perfusion and causes tissue damage. When not treated immediately, it can result in severe complications such as muscle necrosis and loss of function of the affected limb.
Aim: To report a case of compartment syndrome in the forearm treated with a skin graft. Method: This study will describe a patient treated at the Hospital Universitário São Francisco na Providência de Deus (HUSF), located in Bragança Paulista – SP, Brazil.
Case Report: A 45-year-old male patient, admitted after a Bothrops snake bite to the left forearm, presented with intense pain, progressive edema and local signs of envenomation. Despite antiophidic serum and analgesia, he developed signs of compartment syndrome, requiring emergency fasciotomy. The surgery provided pressure relief, with satisfactory progress and subsequent skin grafting to close the wound. After complete recovery, the patient showed minimal functional loss and no further complications.
Conclusion: The evolution of the case presented was favorable, highlighting the importance of a rapid and effective approach to snake envenomation. Early administration of anti-snake serum and appropriate management of complications, such as compartment syndrome, were fundamental in preserving the function of the affected limb and avoiding sequelae. Fasciotomy and skin grafting, followed by post-operative care, ensured the patient's complete recovery, as he maintained functionality and had no further complications. This case reinforces the need for vigilance and multidisciplinary intervention to optimize outcomes in serious snakebites.
